By signing below, I, the undersigned student, voluntarily agree to the following terms and conditions in consideration for being permitted to participate in yoga classes, workshops, or other activities provided by Kula, LLC. (herinafter referred to as "the Studio").
Assumption of Risk
I acknowledge that yoga involves physical movement, breathwork, and postures and that participation carries the risk of physical injury, including, but not limited to, muscle strain, sprains, falls, or more serious injury. I understand that it is my responsibility to consult with a physician regarding my ability to participate in yoga classes before enrolling.
I represent and warrant that I am physically fit, and I have no medical conditions that would prevent my full participation in classes. I understand that it is my responsibility to listen to my body, modify my poses when necessary, and notify the instructor of any limitations or concerns.
Release of Liability
I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby waive, release, and discharge Kula, LLC., its owners, instructors, employees, agents, contractors, and affiliates from any and all claims, demands, causes of action, or liability for injury, disability, death, loss, or damage to person or property arising out of or related to my participation in yoga classes or use of the Studio's facilities.
This waiver applies to any claims based on negligence, action, or inaction of Kula, LLC., and its affiliates, to the fullest extent permitted by Tennessee Law.
COVID-19 and Infectious Diseases Acknowledgement
I acknowledge the contagious nature of COVID-19 and other infectious diseases. I voluntarily assume all risks related to exposure or infection and release Kula, LLC., from any liability arising therefrom.
Photography and Marketing Release
I understand that the Studio may occasionally take photos or videos during classes for promotional purposes. I grant permission for my image to be used unless I notify the Studio in writing that I decline.
Policies and Conduct
I agree to follow the Studio's policies, respect instructors, and fellow students and act responsibly while participating in classes, workshops and other activities. I understand that inappropriate behavior may result in dismissal without refund.
By signing below, I confirm that:
I have read and fully understand this waiver and release agreement.
I am of legal age (18) or have my parent/legal guardia's consent if under 18.
I voluntarily agree to the terms stated above.
